Certificate of Relief from Disabilities Investigation and Report

A certificate of relief from disabilities is meant to reduce the automatic rejection and community isolation that often accompanies the conviction of crimes and contributes to the complete rehabilitation of first offenders and their successful return to responsible lives in the community.

A criminal conviction can have a number of collateral consequences that could potentially hinder eligibility for financial aid for education and housing, for the right to vote, and in finding employment. Any one of these obstacles make it difficult for an offender to re-integrate into society.

Offenders who have been convicted of a crime or of an offense, but have not been convicted of more than one felony are eligible and can apply for this certificate. If a Certificate of Relief is requested for a conviction whereby the person is currently on probation, the Certificate will only be temporary.

INSTRUCTIONS

It is suggested you review the attached material to determine if you are eligible to apply for a Certificate of Relief or a Certificate of Good Conduct

  1. Fill out and have notarized the one copy of the application for the
  2. Fill out four (4) copies of the actual
    1. (There needs to be four (4) original signature It is therefore best to provide the court with four (4) copies).
  3. You may have to contact the arresting Police Agency to obtain some of the
  4. Submit the Application and the four (4) copies of the certificate to the court from which you were sentenced or to the parole board (Whichever is applicable).

DO NOT SUBMIT IT TO THE PROBATION DEPARTMENT

The court may ask the Probation Department to conduct a brief investigation to determine your eligibility for the Certificate.

Please be very specific as to why you are applying for a Certificate as many employers and licensing agencies require the Certificate address very specific questions.
